Sherri Horvat is a Nurse Practitioner and the VP of Nursing/Care Management Programs, Lead Diabetes NP for Circadian Care. Circadian provides patients with care teams to manage diabetes through cellularly-enabled glucometers or CGM which allows the team to assist the patient in achieving glycemic consistency and healthy lifestyle changes.
