Individuals with diabetes come from all walks of life, and often, those who experience many social determinants of health (SDOH) barriers and implicit biases are among them. This population has poorer health outcomes and is not provided the tools, education, and technology that others are. This poster describes the development and outcomes of a telehealth program that reached out to people with diabetes in the poorest county in California (Fresno County) and provided a cell phone-enabled glucose meter to collect data. In addition, an RN, CDCES, engaged with the participants regularly to provide diabetes self-management education and support (DSMES).
