(M14) A Comprehensive, Coordinated Approach to Diabetes Care, Education, and Quality Improvement

In 2024, University of Maryland Baltimore Washington Medical Center launched the interdisciplinary Diabetes Care Collaborative to close gaps in diabetes care. This session describes how the Collaborative leads process improvement in education, care transitions, medication and technology access, and prevention. Diabetes NPs embedded in inpatient units, the Transitional Care Center, and primary care provide medical management and education. CDCESs deliver individualized self-management education, technology support, and help identify high-risk patients. SDOH screening links patients to care managers and community resources. Early outcomes include marked reductions in hospitalizations and ED use, along with clinically meaningful A1c improvements among primary care patients. The presenters will share strategies and lessons to support replication.
