This session highlights how a DCES implemented a structured, group-based gestational diabetes education program as an insurance add-on benefit to improve access, continuity of care, and sustainability at scale. Using the Sweet Peas program as an example, attendees will learn how prevention-focused, culturally responsive education supports risk awareness during pregnancy and prepares individuals for postpartum transitions. Participants will leave with practical strategies to adapt and replicate this model across practice settings, align programs with covered benefits, and support long-term health while reducing future diabetes risk.

Describe education and support needs during pregnancy and postpartum for individuals with gestational diabetes.
Apply a support group model to address glycemic management and postpartum expectations in gestational diabetes care.
