(S13) Reimagining DSMES Services: Complementing the 4 Critical Times with Continuous, Peer-Driven Support

Diabetes is a complex condition that is largely self-managed. Successful self- management to achieve both short- and long-term health goals is highly dependent on learning and carrying out self-care behaviors in daily life. Ongoing support from the clinical team, family members, and peers (in-person or virtually) is critical to successful diabetes self-management that can be sustained over time. In this presentation, we explore the interrelated role of diabetes self-management education and support services with peer support and propose a synergistic approach to bringing the two together to help people with and at risk for diabetes improve their health and wellbeing.

Learning Objectives:

At the completion of this activity, participants should be able to:
  • Describe how DSMES and peer support are distinct but complementary and the evidence supporting both.
  • Discuss strategies for complementing the 4 critical times to refer to DSMES with continuous peer support using the Identify-Configure-Collaborate Framework.
  • Describe how DCES reimagine DSMES through just-in-time care while evolving peer support into behaviorally driven, scalable engagement model.
