Transitions from the hospital to the community pose significant challenges for patients with diabetes, particularly those who are newly diagnosed. Uncertainty surrounding the management of lifestyle changes, new medications, and emerging technologies can elevate the risk of premature readmission. In this session, participants will identify barriers that affect both nursing staff and patients in teaching and learning diabetes self-care during the transition from hospital to community. Additionally, attendees will learn how one healthcare system leveraged technology and other resources to strengthen the bedside nurse’s capacity to deliver essential diabetes education before hospital discharge.

At the completion of this activity, participants should be able to:
Recognize potential barriers within the hospital care system in preparing patients with diabetes for transitions in care.
Identify how to use technology to streamline patient education available to nurses when providing diabetes education in preparation for discharge.
Implement approaches that empower bedside nurses to initiate and reinforce diabetes education for patients prior to hospital discharge.
