Continuous Quality Improvement (CQI) is often treated as a reporting requirement rather than a strategy for program sustainability. This session addresses the challenge many DSMES teams face when CQI activities feel disconnected from daily workflows, team roles, and long-term program stability. Participants will explore how team-based CQI can strengthen collaboration, improve program performance, and support sustainable DSMES infrastructure. Using a case study approach, the session demonstrates how CQI data can guide meaningful improvement efforts, inform leadership decisions, and reinforce the value of the multidisciplinary care team. Attendees will gain practical strategies to use CQI as an ongoing tool for sustaining and strengthening DSMES programs. This session will be collaborative and may include interactive elements.
