The effectiveness of DSMES in improving diabetes outcomes is widely recognized yet many people with type 2 diabetes never receive any diabetes education. A recent survey of patients with type 2 diabetes in our health care system showed that up to 50% had never received DSMES. To help fill this gap and attempt to provide DSMES to harder to reach populations, we implemented a free digital diabetes education chat and offered it to patients with T2DM in our system receiving A1C care gap closure messages. Enrollment data was analyzed to assess patient engagement in various demographic groups. Patient satisfaction and impact on outcomes and self-care confidence were also assessed.
