(F05) Transforming DSMES Through Community-Based Care Models and Continuous Connected Care

People at risk for or living with diabetes often face barriers that limit access to high-quality education and ongoing support. This session highlights an innovative model that brings DSMES directly into communities, meeting individuals where they live, work, and gather, and integrates continuous connected care to enhance follow-up, self-management, and outcomes. Presenters will describe how pharmacists, DCESs, and Community Health Workers collaborate to deliver person-centered care, address social needs, and strengthen continuity between visits. Attendees will gain practical strategies to implement community-based DSMES, leverage interprofessional teams, and incorporate remote care coordination to improve engagement and program sustainability.

Learning Objectives:

At the completion of this activity, participants should be able to:
  • Describe how community-based DSMES models expand access, address social needs, and improve engagement in diverse populations.
  • Analyze how continuous connected care strengthens follow-up, supports person-centered self-management, and enhances DSMES program sustainability.
  • Apply interprofessional strategies to implement scalable DSMES models using CHWs, pharmacists, and technology-supported care.
