D12 - Implementing Lifestyle Medicine for People with Diabetes in a Rural Community

Diabetes management is challenging for hospitals and practitioners. These challenges become even more pronounced when they occur in a rural setting. This session examines barriers to implementing lifestyle medicine such as access to grocery stores, lack of transportation, and resistance from a farming culture. The presenter also addresses how to gain support from colleagues to create a multidisciplinary team approach. Attendees will learn how to help patients shop and prepare simple, whole-food-based meals from Dollar General and learn how to use practices from the Lifestyle Medicine pillars (stress management, physical activity, and sleep) to create a stepwise approach to effect change.
