(P305) Advancing Diabetes Care Pre-Surgery with the CDCES-Led "Operation: A1c" Program

Suboptimal glucose management before elective surgeries can greatly deteriorate rehabilitation outcomes, as well as place an unfavorable burden of cost on the healthcare system providing care for people with diabetes. Through a collaborative, multidisciplinary clinic led by the CDCES professional, the "Operation A1c" glucose optimization program demonstrates rapid improvement in pre-surgery glucose management with exceptional outcomes, elevated patient engagement and self-efficacy, and decreased cost to the healthcare system. This poster details strategies for implementing a pre-surgical multidisciplinary program and the development, training, and implementation of CDCES-owned protocols for medication management and titration.
