(P207) Integration of Community Health Workers to Address Disparities in Diabetes Care

Social determinants of health (SDOH) have disproportionately impacted specific areas and BIPOC communities for centuries. In St. Louis, Missouri, demographics such as a street or zip code convey the impact of disparities, specifically regarding the life expectancy of communities. This poster presentation will examine the impact of disparities on the health of vulnerable communities, discuss the importance of assessing community members/patients for SDOH, and identify innovative partnerships and solutions to address these barriers to care.
