Diabetes Self-Management Education and Support (DSMES) provides critically important information and support for behavior change for people with diabetes, leading to improved health outcomes. However, access to DSMES can be challenging. This poster addresses how diabetes care and education specialists in Kentucky are utilizing a variety of telehealth options for the delivery of DSMES that overcome barriers such as transportation, childcare, shortage of local diabetes educators/ DSMES services, and low broadband access. The poster authors share results from these telehealth modalities and explore further applications and considerations for telehealth utilization.
