New York City lacks accessible Diabetes Self-Management Education and Support (DSMES) programs in neighborhoods with high rates of elevated A1C levels, such as the Bronx, Queens, and Brooklyn. Clinical organizations in these areas face challenges in navigating the complex accreditation process due to administrative burdens, limited resources, and a lack of technical expertise. To address this, the NYC Health Department launched a DSMES Umbrella Accreditation program, which provides technical assistance to clinical organizations seeking DSMES accreditation through an umbrella model. This poster discusses key successes, challenges, and best practices in providing technical assistance to enhance equitable access to DSMES services.
