(P306) Developing a Nursing Diabetes Clinical Support Team to Advance Inpatient Diabetes Management

Hyperglycemia can lead to multisystem organ failure and increase the risk of infection and disability post-hospital discharge. Hypoglycemia may increase hospital length of stay and raise the risk of mortality. These complications may be avoided if a support team had specialty training to identify and escalate those at risk for blood glucose-related events before consequences occur. A Diabetes Nurse Clinical Support Team is a nurse-driven initiative designed to enhance the care of patients with diabetes by providing nursing members with comprehensive knowledge and skills through education. This poster describes the research, design, build, and achievements of the inpatient Diabetes Clinical Support Team at Rush University Medical Center.
