The University of Nebraska Medical Center’s Diabetes on Track project is a program funded by a grant from the Diabetes Care Foundation of Nebraska to transform diabetes care and education in rural Nebraska. Our poster provides a wrap-up of 3 years of Quality Improvement work. It reviews the initial findings of 2 rural Nebraska sites, identifies which team members became the driving forces of improving diabetes related metrics, illustrates the processes that worked, reviews the data they collected to measure progress, describes how challenges were navigated, and explains how the sites plan to sustain the improvements.
