Lifestyle changes needed to manage diabetes may disrupt home life, with family members sometimes being unsupportive. This poster reviews how the SHINE program at the Oklahoma City Indian Clinic takes a family-centered approach to T2D management. It focuses on education, nutrition, physical activity, and self-management involving patients' support systems. Key strategies include CGM and weekly education. The SHINE program improved Hemoglobin A1C levels, reduced body fat and screen time, and lowered intake of sugary drinks. Proving the value of culturally tailored, family-based care can help individuals learn how to self-manage T2D.
