(P314) Impact of an Interdisciplinary Approach for Sustained Improvement in Inpatient Hypoglycemia

Hospitalized patients on insulin have a 2.5 times higher risk of having a hypoglycemic episode during their hospital admission. One cause of hypoglycemia that has been identified is a mismatch of insulin to meal delivery. This poster explores innovative approaches to reduce hypoglycemia and sustain the reduction over multiple years. The poster authors explore how targeted communication amongst interdisciplinary teams, nurse champion empowerment, and adherence to patient quality protocols can result in positive and sustainable outcomes.
