(P405) DKA Outcomes: Comparing Mortality and Readmissions in UTI vs. Pneumonia Patients

Infection is a well-known precipitant of diabetic ketoacidosis (DKA), responsible for nearly half of all cases. Yet despite this, the clinical community fails to distinguish how different infection types may uniquely influence outcomes. Amid the COVID-19 pandemic, viral pneumonia emerged as a major DKA trigger, leading to dramatic surges in ICU admissions and in-hospital mortality. This prompted a critical question: Do specific infections, such as viral pneumonia or different urinary tract infections, alter the trajectory of DKA outcomes, and should we manage these patients differently based on infection type? Surprisingly, few large-scale studies have explored this question in depth. Our research fills that gap. This poster describes a retrospective cohort study across HCA East Florida hospitals, focusing on adults (≥18 years) admitted between January 2020 and December 2023 with DKA and a single concurrent infection—either pyelonephritis, cystitis, bacterial pneumonia, or viral pneumonia. We excluded patients with confounding factors such as immunosuppression, malignancy, or multiple infections to isolate the effect of each infection type on DKA. By using logistic regression models adjusted for age, sex, and race, our analysis seeks to clarify whether specific infections correlate with worse clinical outcomes, particularly in-hospital mortality and 30- and 60-day readmissions. Our poster shares how we stratified patients by infection type, what characteristics we used to adjust for confounding, and how infection-specific DKA may warrant a shift in current clinical thinking. This research proposes a paradigm shift in how DKA should be triaged, risk-stratified, and potentially managed early during hospitalization. This project is more than just a data analysis—it’s a call to action. It challenges the current "one-size-fits-all" approach to DKA management and opens the door to infection-specific care models that could improve survival and reduce readmissions. For clinicians, researchers, and hospital systems alike, these insights may be the missing piece in optimizing outcomes for one of diabetes's most dangerous complications.
