More than 8% of Texans have diabetes. For many, having to make dietary and lifestyle changes, plus the high cost of medication, is overwhelming. This poster describes Baylor Scott & White Health Plan’s BSW Be Well™ with Diabetes program that supports necessary changes, helps manage diabetes, and provides diabetes medications/supplies at no cost. This resulted in increased medication taking, better follow-up with provider visits, and more. The program is managed by a clinical pharmacist who is a CDCES. Eligible members enroll in this incentive-based wellness program, which incorporates the American Diabetes Association (ADA) Standards of Care. Members engage in 1:1 health coaching and are empowered to take an active role in managing their health.
