(P202) Empowering Patients: Breaking Down Barriers to Improve Health Literacy in T2D Management

Low health literacy is a barrier associated with high costs and poor health outcomes. Health literacy is a shared responsibility. Diabetes management encompasses a wide range of skills that may be neglected due to low health literacy, lack of diabetes knowledge, or mistrust of providers. This poster explores interventions to break down the barriers to improve health outcomes for the person with diabetes. Attendees will gain insight on how to implement interventions that consider health literacy to improve self-management of T2D.
