(P626) Utilizing ArcGIS Technology to Deliver Equitable Care for Diverse Populations

This poster reviews the construction of a GIS application centered on diabetes and diabetes-related spatial data utilizing ArcGISa. Pending available data, the GIS app will include census tract and municipal level layers exhibiting where high percentages of communities living with diabetes, prediabetes, foot ulcers, CKD, hyperlipidemia, high blood pressure, and other cardiovascular risk factors are located. Additionally, the app will include demographic data, locations of diabetes-related healthcare services, and drive times to those services, which can be layered over other diabetes-related data. This app informs targeted outreach to specific areas and census tracts within Ocean County.
